Are you looking for an exciting opportunity to join a great team and make a difference to people living with life limiting illnesses?

We are looking to recruit a permanent Assistant Practitioner to work as part of our Outpatient Team. You will assist in the St Andrew’s Hospice Outpatient Department with the aim of helping and supporting patients with a life limiting illness and their carers to remain living in the community for as long as is possible and to maximise their quality of life.

The department is made up of a multi-disciplinary team led by the Outpatient and Community Team Manager with input from the wider team involved in providing inpatient care. The department aims to use a Quality Improvement methodology to ensure services are continually being evaluated to meet the needs of our patients and their carers.

The postholder should have a minimum of 3 years’ experience in a health, education or social care environment and hold an SVQ III in Healthcare. You will plan and prioritise patient caseloads and a range of therapeutic activities and deliver prescribed education sessions to selected groups of patients. You will lead/oversee drop in services within the Outpatient Department and encourage patients and their carers in an active approach to regaining and maintaining personal independence. This requires empathy and sensitivity.
