Outpatient Services

The following services are available for any patient with advanced illness due to a cancer diagnosis, end stage Renal, Cardiac or Lung Disease or an advanced neurological condition such as Multiple Sclerosis, Motor Neurone Disease or Parkinson’s Disease.

We also provide support services for their carers/family members.

The information provided is reflective of our current services and the referral criteria is specific to the conditions mentioned above.

Wellness Together: Monday from 1pm – 3pm
Our Wellness Together programme is designed for patients with a life-limiting condition, providing emotional and practical support, social inclusion and gentle activities. Activities include Music Therapy as well as ongoing support to improve patients’ health and wellbeing and help them to live well with their condition. There is also support available for carers and family members as part of our Wellness Together programme.

People with Parkinson’s exercise class: 2 sessions
Every Monday at 10:30am – 12:00pm and 12:30pm – 2pm
Movement, slowness, and stiffness can be difficult when you live with PD. Slowness in particular can make you feel weak and tired. Adding regular exercise may improve your overall mobility and quality of life. Our qualified and experienced assistant practitioner uses evidence-based exercise and movements specific to people with PD.

As PD advances, patients often have difficulties with walking, mobility, posture, and balance. These symptoms may improve with exercise and the risk of falls may also decrease. We also deliver regular bite-sized education slots and tips from Hospice staff and guest speakers. If you are living with PD, please speak to your Healthcare Professional and ask to be referred to our class.

Wellbeing Programme: Tuesday & Wednesday groups
From 10.30am – 2.30pm
We work with patients throughout this nurse-led programme to support their practical and emotional health and wellbeing through creativity, education and relaxation and also personal goal setting, enhancing their physical health through chair based exercise – all with the aim of improving patients’ quality of life and to enable them to live with their condition.

Strength & Balance: Tuesdays at 2:30pm and Thursdays at 10am
We deliver a nationally recognised, evidence based Strength & Balance programme. This is led by our Postural Strength Instructor (PSI) and adapted for patients living with a life-limiting illness.

Fatigue Management: Thursdays at 1pm
Chronic Fatigue Syndrome (CFS) can be common with illness progression. Our Fatigue Management course is delivered by specialist trained staff who explore the causes and effects of fatigue as well as provide patients with support and information. The aim of the programme is to help
patients manage fatigue, conserve energy and help improve their quality of life. The programme is delivered over 6-8 weekly sessions. Patients will be provided with diaries to record their weekly tasks, fatigue levels and feelings. Throughout the course, we will discuss weekly topics and concerns and take part in gentle movement activities and relaxation.

One-to-one with Outpatient Nurse Specialist – For Patients and/or Carers
Providing a private confidential space, a listening ear and time to talk, aiming to offer advice and emotional/practical support.

One-to-one with Outpatient Assistant Practitioner
Specialising in physical movement specific and supportive to the patient’s condition. This service is available to patients who have poor body
strength and balance. This is a short term solution to give them time and build confidence to enable them to transition into group sessions.

Complementary Therapy for Patients and/or Carers – By appointment
Therapies include; Therapeutic Massage, Reflexology, Aromatherapy, Reiki and Indian Head Massage. These therapies aim to provide relaxation, reduce tension, stress and anxiety and contribute to a sense of wellbeing.

Carers’ Café – Every Monday between 12noon – 3pm
Our Carers’ Café is designed to support families & carers affected by someone living with a life-limiting condition and advanced illness. It’s an opportunity for caregivers to relax, unwind, meet other carers, as well as our team who can offer information, support & advice. There’s also an opportunity to receive a Complementary Therapy or participate in Adapted Tai Chi.

Adapted Tai Chi – Every Friday from 11am
For patients with specific advanced illness and their carers. This is a gentle mind and body programme, adapted for the individual and can be performed in a seated, standing or supported standing position. Adapted Tai Chi can improve mobility & balance, reduce pain, enhance overall wellbeing, improve posture, improve cardiovascular and respiratory health and reduce stress.

Bereavement Café – Every Second & Fourth Friday: 12.30 – 2pm
A secure, comfortable environment where you can chat about how you are feeling and connect with other people who are grieving.

Multiple Sclerosis (MS) Support Group – Third Thursday of each month from 12noon – 2pm
Run in partnership with MS Society Lanarkshire.
